Психологические аспекты раннего хирургического вмешательства при врожденной челюстно- лицевой патологии

Abstract

The paper discusses the influence of early surgery in conditions of congenital craniofacial anomalies on the formation of the personal qualities of a child that are necessary for normal communication, on the development of relations with the mother, on the formation of the communicative behavior mechanism in childhood and in the teens. The authors show that, together with the solution of medical problems (elimination of defects in the development of the mouth cavity and teeth, chronic ear infections, etc.), that provide the necessary basis for normal speech communication, operations at an early age have as their aim the development of communicative activity, emotional reactivity in communication, and communicative confidence as the grounds for the social adaptation of the personality of the child. However, the temporary aspect of surgery is analyzed in terms of preventing behavioral disturbances and educational difficulties in children of this group in connection with the parents' necessity to overcome their feeling of guilt or their excessive parental care, as well as stigmatization in the society of children with ua non-standard appearance” that is aggravated by speech defects.

В статье обсуждается влияние раннего хирургического вмешательства при тяжелой врожденной челюстно-лицевой патологии на формирование качеств личности ребенка, необходимых для осуществления полноценной коммуникации, на развитие взаимоотношений в диалоге с матерью, на становление механизмов коммуникативного поведения в детстве и подростничестве. Авторами показано, что наряду с решением медицинских проблем (устранение дефектов развития полости рта и зубов, хронических инфекций уха и т.д.), обеспечивающих необходимую базу для нормальной речевой коммуникации, операции в раннем возрасте направлены на развитие коммуникативной активности, эмоциональной реактивности в общении, коммуникативной уверенности, как оснований социальной адаптации личности ребенка. Наряду с этим, временной аспект хирургического вмешательства рассмотрен в контексте предупреждения возникновения поведенческих нарушений и трудностей в обучении у детей данной группы в связи с преодолением родительского чувства вины или чрезмерной родительской опеки, а также стигматизации в обществе детей с «нестандартной внешностью», отягощенной речевыми дефектами.
