Abstract: | It is spent the glandular-stromal morphometric and cyto-nucleometric analysis of endometrium, activity of nucleolar organizers and ploidy of epithelial s nuclei of endometrial glands is investigated at initially revealed, recurrent and atypical endometrial hyperplasia in 145 samples of a mucous membrane of the uterus, received in a course aspirating biopsy of endometrium or separate diagnostic currettage walls of a cavity of a uterus and cervix uteri. Authentic differences recurrent endometrial hyperplasia from non-recurrent hyperplasia on a number morphometric indicators and their aggravation are established at atypical endometrial hyperplasia. At recurrent a suffering variant the increase in a share of a glandular component of endometrium, increase glandular-stromal factor, maintenance reduction stroma, the big sizes of glandular epithelial s cells, their nuclei, smaller values of the nucleo-cellular relation and the factor of the form of a nuclei, increase in quantity of nucleoli and argyrophilic granules at the expense of increase out-nucleoli shares, higher maintenance of a genetic material (average index increase ploidy of glandular epithelial's nuclei), strengthening «proliferative activity» and increase «an index dis-differentiation» is noted. The listed morphological parametres probably to consider as prognostic structural markers of development of recurrent endometrial hyperplasia and formations of atypical variants of suffering. Проведен железисто-стромальный морфометрический и цито-кариометрический анализ эндометрия, исследована активность ядрышковых организаторов и плоидность ядер эпителиоцитов эндометриальных желез при первично выявленной, рецидивирующей и атипичной эндометриальной гиперплазии в 145 образцах слизистой оболочки матки, полученных в ходе аспирационной биопсии эндометрия либо раздельного диагностического выскабливания стенок полости матки и цервикального канала. Установлены достоверные отличия рецидивирующей гиперплазии эндометрия от гиперплазии с нерецидивирующим течением по ряду мофрометрических показателей и их усугубление при атипичной гиперплазии эндометрия. При рецидивирующем варианте страдания отмечено увеличение доли железистого компонента эндометрия, возрастание железисто-стромального коэффициента, уменьшение содержания стромы, большие размеры клеток эпителиоцитов желез, их ядер, меньшие значения ядерно-клеточного отношения и фактора формы ядра, увеличение количества ядрышек и аргентаффинных гранул за счет возрастания внеядрышковой доли, более высокое содержание генетического материала (увеличение среднего показателя плоидности ядер эпителиоцитов желез), усиление «пролиферативной активности» и возрастание «индекса дисдифференцировки». Перечисленные морфологические параметры возможно считать прогностическими структурными маркерами развития рецидива гиперплазии эндометрия и формирования атипичных вариантов страдания. |
