Структурно-функциональные изменения сердца у больных артериальной гипертонией в условиях тюменского севера, взаимосвязь с данными СМАД

Abstract

393 males aged 20 to 59 years were studied for the structural changes of the heart in correlation with the data of ambulatory blood pressure monitoring (ABPM). The study group included 220 patients with arterial hypertension (AH) working in shifts in the Far North. A 24-hour blood pressure profile in the AH patients from the North was characterized by a 'plain* curve of rhythm and bias in hyperbaric load towards night hours. In the North hypertensive subjects had prevailing symptoms of hyperreactivity of sympathetic nervous system and development of stress-induced AH against a background of significant increase in left ventricular (LV) mass and LV mass index. The study showed prevalence of concentric LV hypertrophy in the patients with mild-to-moderate AH. In healthy subjects a concentric type of myocardial remodeling was observed, and the values of LV mass and LV mass index were higher than in the comparison group. In the hypertensive subjects from the North there was a positive but weak association between LV mass, LV mass index and ABPM characteristics. The revealed changes show that there are more complicated mechanisms of the development of structural changes of the heart in AH patients during the adaptation to the extreme conditions of the North that include apart from blood pressure, all the spectrum of neurohumoral and trophic factors at plasmatic and tissue levels.

Проведен сравнительный анализ структурных изменений сердца во взаимосвязи с данными СМАД у 393 мужчин в возрасте 20-59 лет. Группу наблюдения составили 220 пациентов с АГ, работающих в режиме вахты на Крайнем Севере. Суточный профиль АД у северян с АГ характеризовался «плоским» типом ритма и смещением гипербарической нагрузки на ночные часы. В условиях Севера у лиц с АГ преобладали признаки гиперреактивности симпатической нервной системы и формирование стресс-индуцированной АГ на фоне значимого увеличения ММЛЖ и ИММЛЖ. Показано преобладание концентрической ГЛЖ у больных АГ 1,2 ст., у здоровых лиц наблюдался концентрический тип ремоделирования миокарда при величинах ММЛЖ и ИММЛЖ, превышавших показатели группы сравнения. В группах северян с АГ отмечена значимая, положительная, но слабая корреляционная зависимость ММЛЖ и ИММЛЖ с параметрами СМАД. Выявленные изменения свидетельствуют о более сложных механизмах развития структурных изменений сердца у пациентов с АГ при адаптации к экстремальным условиям Севера, включая, помимо АД, весь спектр нейрогуморальных, трофических факторов на плазменном и тканевом уровнях.
