Современные принципы и методы местного лечения при острых гнойно-воспалительных заболеваниях челюстно-лицевой области

Abstract

Subject. A literature review is devoted to the urgent problem of maxillofacial surgery and surgical dentistry - the effectiveness of local treatment of patients with acute purulent-inflammatory diseases of the maxillofacial localization. The purpose of the study is to study the materials of publications devoted to modern principles and methods of local treatment for acute purulent-inflammatory diseases of the maxillofacial region. Methodology. A review of literature sources is conducted and the principles and methods of local treatment are analyzed depending on the stages of the wound healing process, their effectiveness and disadvantages are described. Results. Today, more preferred main methods of local treatment for acute purulent-inflammatory diseases of the maxillofacial region are active drainage and sorption-application therapy, which lead to the creation of adverse conditions for the life of microorganisms in a purulent wound. However, their main drawback is the use only in the first phase of the wound healing process. As for the medicines used to influence the purulent wound, among them there is not a single universal remedy that affects all the links of the purulent wound process. Conclusions. The presented results of the review showed that none of the existing methods of local treatment of acute purulent-inflammatory diseases of the maxillofacial region today fully satisfy specialists, therefore, the relevance of research aimed at developing new methods of local treatment of purulent infection and their socio-economic significance obvious. In this regard, there is an urgent need to develop affordable and, at the same time, effective approaches to the treatment of this category of patients that meet modern requirements. In this regard, the use of the achievements of gene-cell therapy in combination with classical methods will, in our opinion, significantly increase the effectiveness of local treatment

Предмет. Представлен обзор литературы, посвященный актуальной проблеме челюстно-лицевой хирургии и хирургической стоматологии –эффективности местного лечения пациентов с острыми гнойно-воспалительными заболеваниями челюстно-лицевой локализации. Цель исследования – изучить материалы публикаций, посвященных современным принципам и методам местного лечения при острых гнойно-воспалительных заболеваниях челюстно-лицевой области. Методология. Проведен обзор источников литературы и проанализированы принципы и методы местного лечения в зависимости от фазы течения раневого процесса, описана их эффективность и недостатки. Результаты. На сегодняшний день более предпочтительными основными методами местного лечения при острых гнойно-воспалительных заболеваниях челюстно-лицевой области являются активное дренирование и сорбционно-аппликационная терапия, приводящие к созданию неблагоприятных условий для жизнедеятельности микроорганизмов в гнойной ране. Однако их главным недостатком является использование только в первую фазу раневого процесса. Что касается лекарственных средств, используемых для воздействия на гнойную рану, то среди них нет ни одного универсального средства, влияющего на все звенья гнойного раневого процесса. Выводы. Представленные результаты обзора показали, что ни один из существующих методов местного лечения острых гнойно-воспалительных заболеваний челюстно-лицевой области на сегодняшний день не удовлетворяет специалистов полностью, следовательно, актуальность исследований, направленных на разработку новых методов местного лечения гнойной инфекции и их социально-экономическая значимость очевидны. В связи с этим встает острая необходимость разработки доступных и, в то же время, эффективных подходов к лечению данной категории пациентов, отвечающих современным требованиям. В этом плане использование достижений генно-клеточной терапии в сочетании с классическими методами позволит, на наш взгляд, существенно повысить эффективность местного лечения
