Радиочастотная аблация при вторичном опухолевом поражении печени

Abstract

The aim of our investigation is an estimation of possibilities and results of radiofrequency ablation of liver's metastases. RFA metastases of colorectal cancer in the liver was made to 162 patients, in age of 32 to 82 years old. In all there were 258 centers subjected to influence. There were no lethal outcomes and complications, which needed surgical intervention or using intensive therapy. On our evidence, the percutanceous radiofrequency ablation is an effective and save method of minimally invasive influence on metastatic tumours in the liver, which has no an evolution of serious complications under the correct implementation. In our opinion, the results of the local influence on the metastatic centers can be improved at the expense of combination of RFA with different variants of chemotherapy for the warning of both relapses of the disease and repeated metastasis, and for perfection of the technique of interference.

Метастатическое поражение печени при колоректальном раке остается одной из важнейших проблем. Основным методом лечения является хирургический, однако, радикальное вмешательство может быть проведено не более чем у 15-30% пациентов. Малоинвазивные методы локальной деструкции опухолей все шире внедряются в онкологии, демонстрируя обнадеживающие результаты. Нами проанализированы результаты чрезкожной и интраоперационной радиочастотной аблации у 168 пациентов с метастазами КРР в печень - всего 258 очагов. Оценена безопасность и эффективность метода, а также предложены пути улучшения результатов лечения.
