Диагностика и лечение гнойно-некротических заболеваний наружных половых органов у мужчин

Abstract

On the basis of retrospective analysis of 18 cases of rare diseases of the external genitalia in males, including scrotal carbuncle, hydradenitis of the womb, cavernous and bulbar abscess of the penis, abscess of the median suture of the penis, scrotal abscess and Fournier gangrene were studied the peculiarities of clinical and radiological picture, the diagnosis and treatment of purulent-necrotic diseases of the external genitalia. Most of the clinical diagnostic difficulties were with Fournier gangrene, secondary to the scrotum abscess and purulent hydradenitis of the womb. The main method of diagnosis of purulent-necrotic diseases of the external genitalia was high-resolution ultrasound, which allowed in all cases to establish the diagnosis, stage of the disease, the boundaries of the inflammatory process, to develop the tactics of treatment, cany out fine needle puncture and aspiration of abscess for therapeutic and diagnostic purposes, and in the postoperative period to evaluate the effectiveness of the treatment. The prognosis of purulent-necrotic diseases of the external genitalia depends on their timely diagnosis and treatment.

На основании ретроспективного анализа 18 случаев редких заболеваний наружных половых органов у мужчин, включающих карбункул мошонки, гидраденит лона, кавернозный и бульбарный абсцессы полового члена, абсцесс срединного шва полового члена, абсцесс мошонки и гангрену Фурнье, были изучены особенности клинико-лучевой картины, диагностики и лечения гнойно-некротических заболеваний наружных половых органов. Наибольшие клинико-диагностические трудности наблюдались при гангрене Фурнье, вторичном абсцессе мошонки и гнойном гидрадените лона. Основным методом диагностики гнойно-некротических заболеваний наружных половых органов было высокоразрешающее ультразвуковое исследование, которое позволило во всех случаях установить диагноз, стадию заболевания, границы воспалительного процесса, выработать тактику лечения, осуществить тонкоигольную пункцию и аспирацию гнойника с лечебно-диагностической целью, а в послеоперационном периоде - оценить эффективность проводимого лечения. Прогноз при гнойно-некротических заболеваниях наружных половых органов зависит от их своевременной диагностики и лечения.
